UpsertIssuedActionInput
An action issued by a trusted API client. The caller generates the action id, so a retry updates the same row. While the action is OPEN a retry replaces the payload fields; once the student has seen it, or it has ended, the only accepted change is state CANCELLED. type, version, and targetUserId cannot change under an existing id. Issuing a type requires the caller's tenant to hold that type's permission on urn:platform, for example signals:issue_action:redirect_on_launch.
